  All-ceramic Crowns: A New Approach to an Old Problem  
     
 

The porcelain fused to metal crowns as an alternative to teeth whitening Houston dentist have been using the technique and it has ruled the dental field for over 25 years. They have become the workhorse restoration of choice for the typical general dentist. Even though the advent of porcelain covered metal underneath crowns was an improvement over the full gold crowns, it still had some serious drawbacks.

The metal based porcelain crown may look acceptable in the dental office or at home but under the natural sunlight or fluorescent light it changes enough to look unnatural. This phenomenon is called metamerism (when a crown shade varies in different lighting conditions). You might think it is your imagination, but it is not. While natural teeth transmit some of the light inside through due to their crystal-like properties, the metal does not transmit any. This makes it impossible to have a translucent restoration that mimics the translucency of natural teeth. The results are a subtle variation in appearance and opaque look. Enough to make a metal-based crown look different or fake to the human eye under various lighting conditions.

Houston orthodontists will also tell you that more importantly metal-based crowns tend to develop a dark line around the gum line after a few years. The metal inside the porcelain/metal crown is what causes this dark black line to show at the edge of the crowns.

In order to eliminate the unnatural appearance of the metal-based crown, the dental field experimented with all porcelain crowns. However, they proved to be too brittle to withstand the forces created by jaw muscles during normal chewing. An all porcelain crown is not as strong as a metal based crown therefore, it is usually only used on special occasions.

In the quest to provide improved aesthetics and function to increasingly discriminating tastes, cutting edge cosmetic dentists turned to new and exciting materials. The zirconium-based all ceramic was introduced to the dental community in the early 1990s. Combined with the CAD-CAM technology (Computer Aided Design/ Computer Aided Manufacturing), this new metal-free substructure produces natural looking crowns with strength that surpasses those of a metal-based crown, yet provide the aesthetics of all-ceramic. Using the latest state-of-the-art 3D scanner and a very advanced virtual reality CAD/CAM software program, a replica of the prepared natural tooth is created virtually. The data file is received by a computer-controlled fully automated milling machine. The milling machine uses a solid block of zirconium and a sapphire stylus that drills, carves and creates a substructure with great accuracy. Special porcelain is then added.

A significant reported advantage of the metal-free porcelain crown is its fluorescent properties which are similar to that of the natural teeth and eliminate problems with metamerism. Simply put, a new all-ceramic crown is aesthetically superior to any other type of crown. You just cannot get the same finish and natural living look with a traditional metal-based crown. However, the latter is an established solution. This is probably why it is still commonly used. In addition, providing metal-free, all-ceramic crowns requires a higher level of training and expertise by the cosmetic dentist.

All-ceramic crowns maintain a translucency that makes them hard to tell from natural teeth. Without metal, the occasional problem of the dark blue line at the edge of the gums is eliminated.

This can allow dentist to place the edge of the crown above the gum line if indicated, which can be healthier for your teeth and gums.   Alternatives to Natural Teeth  
     
 

People lose their teeth for many reasons. Of course, Houston cosmetic dentist agree, lack of proper oral hygiene plays an important role in preservation of our teeth. There is no substitute for proper oral hygiene and routine dental check up.

  I’ve Got a Feeling we’re Not in Kansas Anymore…

There are things in life that we look forward to and then there are things that we can do without. For most people a visit to the dentist is in the latter category. Who looks forward to getting an injection in the mouth followed by getting his/her teeth drilled while keeping the mouth open for quiet some times? In case of tooth extraction, the experience is far more dreadful.

Our health care cost have been rising at a significantly faster pace than our income. Over the past decade, the annual cost of health care coverage has risen 131 percent, according to Kaiser Permanente. Many economists believe that increases to the employer portion of health care costs have and will continue to reduce workers’ wages over time.

Has your household income more than doubled in the last 10 years?  According to the United States census bureau, the median household income of America has increased from $38,771 in 1967 to $50,233 in 2007. That is less than 30 percent over the last 40 years and less than 7 percent over the last decade (as opposed to 131 percent). This imparity in growth rate between health care cost and average American income cannot go on indefinitely.

Our world is changing and we must evolve to maintain our standard of living. The yellow brick road is getting fuzzy. How can we break this cycle? Is there any minor adjustment we can make to change the inevitable? I, Dr. Farid Noie believe there is. We have to become more efficient and proactive. There are things that we have little or no control over. We cannot eradicate corporate and financial sector greed.  There is little we can do to keep our politicians immune from playing politics with our future and resources. There are however, a few things that we have control over.

Together we can control the rising cost of sick care by being proactive and getting serious about practicing Preventive health and oral care. Together we can be more green conscious. We can be more energy and fuel conscious. Our collective nationwide minor modifications can produce amazing results. We can once again remind the world what is so special about being American.

Historically, when the chips are down, we have always delivered. We did it during the industrial revolution. We did it during the nightmare of World Wars I and II and the battle of Normandy. We can do it again against current global crisis as the leader of the free world.

In all my years of practicing restorative, surgical and rehabilitative dentistry including dental implants Houston patterns have always been consistent. Those patients of mine that have adapted routine preventive check-up and maintenance habits that are recommended by American Dental Association (I call them my A-list patients) need far less major, often unpleasant and expensive, corrective procedures than the “I go to the doctor when something goes wrong” group. I hereby offer all of my patients a great incentive in order to encourage them to join our A-list. I guarantee all of my major dental work, including dental implants, 100 percent, for as long as (no expiration date) they remain an active patient on our A List, (even if the damage is caused by accident). I hope that my colleagues adapt a similar policy. We all need to do our share to promote and reward wellness through prevention as opposed to sickness in our community.

  The White Coat Syndrome

Additionally, all the exaggerated horror stories we hear and sensationalized movies about a visit to the dentist add yet more anxiety to the heart and mind of most people says Houston sedation dentist, Dr. Farid Noie. A good patient of mine who is an IRS auditor once said “when I come to see you, I can relate to the taxpayers that come to my office with an audit letter in hand”..

 
     
There are also those with a dental Phobia. A "phobia" is traditionally defined as "an irrational severe fear that leads to avoidance of the feared situation, object or activity". Exposure to the feared stimulus provokes an immediate anxiety response, which may take the form of a panic attack. People with dental Phobia experience a great deal of apprehension and anxiety, usually days before the dental appointment. My phobic patients (who are generally in good health and have stable vital signs) display unusually high heart rate and blood pressure while sitting in my chair. This condition is unofficially called “White Coat Syndrome”.

Unfortunately many people avoid going to dentist, even for routine check up and simple procedures due to this fear. This of course leads to much bigger problems as small dental issues grow to become major complications. Many studies show that people who avoid going to the dentist due to anxiety or phobia, on average spend more time and money at the dentist and have more complications with their teeth.

Only if there were a way to reduce or even eliminate the fear factor from the dental visit! Wait a second, there is a way. It is called Sleep or sedation dentistry. I am surprised that Houston sleep dentistry is not more widely used here. In my opinion, there is no need for people to dread or be awake during dental experience. Any patient with a stable medical history qualifies for dental treatment with a Houston sleep dentist.

There are two methods of administering sedation: oral and intravenous (IV Sedation). The oral methods involve taking a pill that is prescribed about a half an hour before the appointment time to ease anxiety. It is important to note that these methods do not provide pain relief, but instead provide anxiety relief. Oral method is simple. However, because it is dispensed in a specific dose form (and some people may need more or less than others) customization can be more challenging and time consuming.

The other method of sedation is the intravenous sedation. This method has the fastest effects on patients. Most people do not recollect the experience at all and wake up feeling fresh. Many of them are under the impression that the procedure only lasted for a few minutes, even if it in fact it may have lasted for hours. In this method Dentist or Anesthesiologist can deliver just the right dose of sedatives that will have maximum effects in minutes. Additionally, if more sedatives were needed during the procedure it can be easily administered. This procedure is also quickly reversible.
